Reports to: MSC Supervisor
Classification: Full –Time (37.5 hours/week)
Required Responsibilities:
Under the supervision of the MSC Supervisor, the Medicaid Service Coordinator (MSC) will provide individual case management services. The MSC is responsible for developing and providing education programs to address specific client needs.
Qualifications:

Preferred- A four year degree in the field of health or human services and no less than 6 months experience working with the developmentally disabled population or service coordination with any population.

Acceptable- A two year degree in health or human services field with no less than 1 year of successful work experience in direct service with developmentally disabled population. Must be a licensed driver with own transportation, able to relate and interact with diverse individuals coming from different social, economic and ethnic backgrounds.
Example of Required Responsibilities:
- Community outreach to identify eligible individuals.
- Consumer enrollments.
- Develops, implements, and maintains ISP’s based on interviews and assessment of consumer’s strengths and needs, working with both the consumer and their families.
- Provide service coordination, which includes information, outreach, linkage, referral, follow-up, monitoring, and advocacy to the developmentally disabled individuals and their families.
- Ensure that service plans are implemented and recorded in case notes and files.
- Coordinate services and resources for consumers.
- Submit monthly billing and reports to Supervisor on time.
- Maintains caseload of 25 consumers with a maximum of 30 consumers, statistics monthly reports as required by program.
- Conducts home visits to ensure that consumer’s needs are met.
- Assists in periodic case reviews as directed by Supervisor.
- Must have own transportation and possess a valid NYS Driver’s License.
- Must submit to a Criminal Background fingerprinting check by OMRDD.
