Reports to: Director of Aging Services
Classification: Part-Time (20 hours/week)
Required Responsibilities:
Advocate/ensure seniors receive entitlements, provide and coordinate information and referral services as needed with other agencies and significant family members; assist with access to systems; act as resource person to community groups associated with senior services; conduct home visits and follow-up on clients including observing, investigating and reporting conditions needing attention. Work in collaboration with Eldersource.
Qualifications:
Bachelor’s degree in Human Services; Bi-lingual Spanish; Experience working with elders a plus.
Example of Required Responsibilities:
- General knowledge of area services for elders, Monroe County Social Services, Social Security and Mental Health Systems
- Knowledge of political and social issues affecting elderly population
- Must possess counseling skills and techniques
- Ability to interpret community resources to clients and address specific needs of geriatric clients
- Ability to access individual and family situations, effectively solve problems and respond in a crisis
- Must demonstrate ability to communicate effectively, both orally and writing
- Maintain accurate case notes
- Complete monthly and statistical reports
- Complete reports, case notes, data entry onto on-line computer data base.
- Demonstrate computer ability and have use of a vehicle to conduct home visits.
